Please Read This First:
The possibility of grant awards is determined by AHP as we analyze your proposed project. The process starts with your perusal of our website, the submission of your electronic Intent To Apply form, consultation with the State Coordinator. The AHP State Coordinator is available via telephone (928-445-3122 X 31) or on-site to help you design your project and write your Intent. This Intent To Apply form is not a full proposal – only an early indicator of your proposed project and a way to know if AHP can fund it. Be as brief as you can.

If you do not wish to submit this form on-line, phone or fax a printable version.
